I certify that I have read and understand all terms and conditions in this contract for training and, hereby waive and release Jennifer Mann dba LASFD, it’s agents and employees from any and all liability of any nature, including but not limited to any illness (including Covid19), injury or damage which myself, a member of my family, or my dog may suffer while participating in an LASFD training program. If necessity for veterinary treatment arises while pet is in the care of LASFD, we reserve the right to have treatment rendered by a veterinarian of LASFD choosing. All costs associated with that treatment shall be the responsibility of the dog’s owner. As the owner and responsible party, I hereby indemnify and hold Jennifer Mann dba LASFD forever harmless for any illness, injury and / or damage to persons or property caused by the dog / owner during and after the training program.

We enjoy seeing your pets’ progress and so do others! From time to time, videos, pictures, voice and testimonial materials may be used strictly for educational and advertising purposes and to help you with your progress. I hereby authorize Jennifer Mann / L.A. School for Dogs to use pictures and video of me (or my dogs and family) taken in a photograph, digital image, videotape, motion picture, and/or testimonial (written words). The undersigned releases Jennifer Mann / L.A. School for Dogs, its agents or employees, as well as any and all users and exhibitors of said pictures and videos, from any and all claims, demands, accountings, and causes for which they may be used pursuant to this Consent and General Release. It is also my understanding that I will receive no compensation for their use.
